**Please note that Chinook Winds Greyhound Rescue does not charge a fee to "process" your application, nor do we require a "deposit" with your application.**

(Please be patient as the download can take a few minutes.) 2. Complete all three pages of the application form. 3. You can either mail or fax your completed application form, send it as an email attachment, or telephone an Adoption Representative for instructions. By mail: ALL AREAS:

By email: Save the application form as a Word Document (.doc). Fill out all three pages of the form online and email to an Adoption Representative as an attachment. As the original signed application form is required for our files, please make a hard copy of the completed form, sign it, and give to the Adoption Representative at the time of your home visit. 4. An Adoption Representative will be in touch with you upon receipt of your application form. If you have not received a call within 3-5 days of sending your application in, please telephone or email your nearest Adoption Representative to ensure your application was received.
